DENTAL IMPLANTS

 

All the common forms of tooth replacement, such as bridges or dentures can be replaced by dental implants. A dental implant is typically a small screw shaped attachment usually made from titanium. Each implant is placed into a socket carefully drilled at the precise location of the intended tooth. Once the bone attaches firmly to the implant, a replacement tooth can be secured to the top of the implant. This creates an initial stability, which over time is steadily enhanced by further growth of bone into microscopic roughness on the implant surface.

The new tooth can look, feel and perform like a natural tooth, are more comfortable than conventional denture because there is no slipping or movements.
This eliminates some of the key worries of dentures and improves self-confidence and it is also very important when teeth are missing the surrounding bones begins to shrink in this cases dental implants can help maintain the shape and structure of the jaw bone.
We need to do a surgical procedure to bury the implant within the jawbone and allow the body to heal naturally. After about 3-6 months, the implants will be strong enough to support the load of new teeth, which is attached to them. During the healing period, you will be provided with temporary "teeth" to allow you to carry on your way of life until the bone completely surrounds the implant. This may be an adhesive-type bridge or a denture.

The success of implants can depend on the ability of the patient to keep their teeth and gums clean. We need to be sure this will take place before implants are placed. We will always advise you on how to care for your mouth to allow you to have implants, and also how to care for your new teeth. Your commitment will be to follow our advice.

The whole procedure to insert the implant into the gum can be done in two different ways: one stage or two stages.

On stage One procedure the gum is cut just once for the surgical integration of implant into the jaw bone. After inserting implant complete with screw into the jaw bone the temporary denture is then connected direct to the screw of the base implant. This kind of denture is replaced with the permanent tooth after about three months when bone and implant have fused together.

With the stage Two procedure this is done by inserting the base implant int o the jaw bone and will take about 6 to 12 weeks for the implant to fuse with the bone. After this period when the bone has grown around the base implant the gum is opened and the special screw with the tooth is then attached.


STEP-BY-STEP PROCEDURE FOR DENTAL IMPLANTS

The implant process normally involves several steps that take place over the period of time from three to nine months. Although there are various implant treatment methods, a typical process often includes:

  1. ASSESSMENT PLANNING

    At initial consultation, after discussing possible alternatives, the dental surgeon will assess the feasibility of providing implant treatment. X-rays may be taken and preliminary models may be prepared. A written plan is usually then formulated detailing the sequence of treatment and associated cost.

  2. IMPLANT PLACEMENT

    A relatively simple minor surgical procedure that can be performed under sterile conditions in dental surgery under local anaesthesia and. If indicated, supplemented by conscious sedation. If after assessment the underlying bone is judged to be deficient, a number of options are available for regeneration. This may be carried out prior to, or at the same time as, the implant placement.

  3. INTEGRATION PERIOD

    Although some implants can be loaded into function immediately or soon after surgery, it is often suggested that a period of three months or more be allowed for the implant to be fully integrate before loading. The patient’s own denture or in some cases a simple adhesive bridge, can be worn during this healing phase.

  4. THE RESTORATIVE PHASE

    Once integrated, the implants may need to be uncovered and can be brought into function with a variety of restorations, ranging from a single crown, small or large bridge, or if preferred, a removable overdenture clipped to the implants. A specialist dental laboratory working closely with the dental surgeon constructs the replacement teeth.

  5. MAINTENANCE

    Following the completion of implants treatment; patients need to clean thoroughly around the implants as instructed by the dentist. They should attend their dentist for regular review so that the health of the soft tissue, bone levels and the integrity of the restoration can be checked.

  • You can talk and laugh without embarrassment – it is not possible to tell the difference between natural and replacement teeth

  • Your new teeth are firmly anchored into the bone, there is no need to alter your natural teeth

  • You can eat whatever you want, without any constraints

When it's a time to use implants?

If you are missing one or more teeth you need a replacement, that's when implants come in handy. They are permanently anchored into the jaw and they function as natural teeth. It’s an optimum method of treatment for:

  • Patients with injuries resulting from accidents (ex. road accidents), in which they lost their teeth:

  • Patients that were forced to wear removable dentures so far, which represented a great discomfort for them in the social life.

How the treatment is being done?

To put it simply, first of all it is essential to cut and rise a flap of the site that is to undergo the treatment. Then, a hole is drilled with a special set of drills and an implant introduced. The membrane is sewn back. The treatment is carried out under local anaesthetic. In our dental practice it is a dental surgeon who performs the operation. A correct implant treatment caries no risk for the patient.

Do implants hurt?

Placing the implants requires a small operation. This can be done using a simple local anaesthetic, and sometimes with sedation if you are very nervous. Sometimes the dentist needs to use a general anaesthetic for complex cases. You will not feel any pain at the time, but you may feel some discomfort during the week after the surgery. This is usually due to having stitches, and the normal healing process.

Implant-supported replacement teeth look, feel and function like natural teeth. This means that you can eat and drink whatever you are used to. You can continue smoking as well, if you must. How will I care for my implants?

Your new teeth must be cared for and checked regularly, just like your natural teeth. Brush and floss as recommended by your dentist or dental hygienist. See your dentist every sixth months, or more frequently if so advised

Am I to old for cosmetic dental implants?

No. Any person at any age can have dental implants as long as there is sufficient bone available in which to place the implants. The amount of bone present can be determined by and x-ray or CT scan. If there is insufficient bone present grafting or bone augmentation may be needed.

I have a full denture, is it possible to replace this using implants?

Yes it is. To restore one jaw (upper or lower) normally requires 6 implants followed by 12 units of bridgework at a later stage. While the treatment is in progree the patient can normally wear their denture

What is involved in placing implants?

First, surgery is performed to place the anchor. Surgery can take up to several hours, and up to six months may be required for the bone to grow around the anchor and firmly hold it in place. Some implants require a second surgery in which a post is attached to connect the anchor to the replacement teeth. With other implants, the anchor and post are already attached and are placed at the same time. After the gums have had several weeks to heal, the next step is begun. The artificial teeth are made and fitted to the post portion of the anchor. Because several fittings may be required, this step can take one to two months to complete. Implant surgery can be done either in a dental office or in a hospital, depending upon a number of factors. A local or general anaesthetic may be used. Usually pain medications and, when necessary, antibiotics are prescribed. Your dentist will give you instructions on diet and oral hygiene.

Is the implant procedure painful?

During the procedure, the treatment area is anaesthetized in virtually the same way as for other dental,or cosmetic dentistry procedure. Patients will normally experience only mild discomfort after the procedure. This can be controlled by over-the-counter or prescription medication, and antibiotics. In almost all cases, the patient will return to work, or their regular routine, the next day.

How long do implants normally last?

It has been demonstrated through long-term studies that end osseous implants regularly last for periods up to 25 years and more. The effectiveness and durability of dental implants is often determined by the patient’s lifestyle. For example, those who smoke, have poor oral hygiene, or grind their teeth, will likely have a lower success rate and shorter implant-life

Is there an age factor for implants?

The health of the patient is a far more important factor than the patient's age. Dental Implants have been placed in patients in their teens. Implants have also been placed in patients in there 80s, allowing them a new lease on life. As a result they have achieved better nutritional standards, and appear physically younger. New techniques and advancements in implant dentistry allow the practitioner to augment bone structure in cases where bone was insufficient for placing implants. So, even though you have lost teeth many years ago, or have worn conventional dentures for many years, and have subsequently lost some bone mass, this can be reversed through bone augmentation.
